Fill out Form No. (103) Notice of Enrollment / Termination of the Insured’s Service.
Declaration of the marital status of the pensioner.
Deduction form.
Copy of Civil ID card, nationality certificate and birth certificate for the retired person and his children.
Copy of marriage contracts and divorce certificates for daughters.
Unmarried certificate for an unmarried daughter aged 15 and over.
A copy of the children’s death certificate.
A salary transfer certificate with the IBAN.
Deduction certificates from government agencies (in case of deductions).
Notes:
In the event of a disability, a disability certificate must be presented + a valid disability card.
In the event that the application is submitted through the attorney, a copy of the POA and a copy of the attorney’s civil ID must be submitted.
In the event that there is a period of interruption from work, please submit a statement of the days of interruption from the employer.
In the event of the divorce of the insured whose service ends with entitlement to a social allowance for a single category with children, please present a certificate of divorce.
